4. If new student, previous piano background and experience (lessons,teachers, etc., if any)

5. Does your student have a real piano to practice on (not a portable electric keyboard)? Do all they keys work? When was it last tune (and don't say, "when Jimmy Carter was president!")?

6. Contact information (mailing address and phone)

7. GIVE ME ALL POSSIBLE LESSON DAYS and TIMES FOR YOUR STUDENT (limited to Wed, Thur, Fri). (NOTE: Lessons are held on days when Ferndale Public Schools have class. We follow the local school calendar). Provide accurate info to the 1/4 hour if you can.

For example: Susie can take lessons before 9:00 AM on Thur and Fri

or, between 4:45 - 6:15 on Thur

or, anytime after 4:00 on Wed.

 

8. GIVE ME PREFERRED LESSON TIMES

For Example: Susie can take lessons during any of the above times. However, we have a preference that she stays in the afternoon, with Friday being the best. Our ideal time would be 4:45 - 5:15 Fri.

(Remember, lessons are 30 min)
